Clinical Quality and Patient Safety

Risk Management Standard for Pre Hospital Care in the Ambulance Service

The Trust has achieved the highest level (level 3) of the Risk Management Standard for Pre Hospital Care in the Ambulance Service.

Data from the Trust's adverse incident and near miss reporting scheme is submitted to the National Patient Safety Agency National Reporting and Learning System.

Clinical errors are more often a result of inadequate or dysfunctional systems or practices within the organisation than individual human error. Therefore, the Trust encourages staff to identify and report accidents, mistakes and near-misses so that lessons can be learned and action taken to avoid repeat occurrences. The Trust's clinical incident reporting system is based on a simple Notification of Clinical Incident/Near Miss form to record details. Reporting errors and problems in this way enables the Trust to explore their root causes. To encourage reporting, promptly reported genuine errors rarely result in disciplinary action.
